Description
This is a 58 year old female who was diagnosed with left breast cancer. She elected to undergo bilateral nipple sparing mastectomies. She did not desire to have implants and therefore underwent reconstruction with DIEP flaps.

Description
This is a 54 year old female with a previous history of right breast cancer for which she underwent a lumpectomy and radiation therapy. She developed recurrent cancer in her right breast and decided to undergo bilateral mastectomies. Due to her history of right breast radiation, implants would have been high risk. She opted for breast reconstruction with DIEP flaps so she could use her own abdominal tissue instead of implants. A breast lift was performed at the same time as the flaps. She then underwent nipple reconstruction and tattooing.

Description
This is a 48 year old female who was diagnosed with right breast cancer. She underwent a right nipple sparing mastectomy and DIEP flap reconstruction. She still needs removal of the right breast skin paddle from the flap and minor revision.

Description
This is a 47 year old female with a previous history of left breast cancer, for which she underwent breast conservation (lumpectomy and radiation therapy). She had a significant left breast deformity from the lumpectomy and radiation and the entire left breast was smaller, contracted, and had the nipple pulled outward. She developed a recurrence in the left breast and therefore underwent double mastectomies. Due to the previous radiation and the already distorted left breast, DIEP flap breast reconstruction was her best option. The skin from her abdomen was used on the left breast in order to replace the lack of skin on the bottom of the breast.

Description
This is a 51 year old female who developed right breast cancer. She did not desire implants and had excess fat and skin on her abdomen. She underwent bilateral mastectomies and DIEP flap reconstruction. She had significant abdominal wall laxity and separation of the abdominal muscles, which was fixed and repaired at the time of the DIEP flaps. She had a breast lift performed at a second surgery as a revision.

Case 28
Description
This is a 46 year old female diagnosed with left breast cancer. She elected to undergo bilateral mastectomies, but her left nipple and areola needed to be removed. She first underwent bilateral breast reconstruction with tissue expanders. She needed a completion left axillary dissection after the mastectomy, which was complicated by left arm lymphedema and a chronic infection in her left underarm area. Due to the risk of infection of an implant, she decided to switch the tissue expanders to DIEP flaps instead of implants.

Case 29
Description
This is a 49 year old female with a previous history of breast cancer, for which she underwent double mastectomies and implant reconstruction by another plastic surgeon. She was not pleased with her implant reconstruction result, as they appeared and felt unnatural. The left implant was smaller, higher, and had significant rippling. She desired to replace both implants with her own tissue. She underwent removal of both breast implants and replacement with DIEP flaps. She underwent a second revision surgery to inject more fat into the flaps to give them an even fuller shape and more projection. She then underwent nipple reconstruction and tattooing.

Case 30
Description
This is a 52 year old female with a history of left breast cancer, for which she underwent bilateral mastectomies and implant reconstruction by another plastic surgeon. She needed radiation therapy to the left breast. She developed left arm lymphedema and recurrent infections of the left breast. These infections needed to be treated multiple times with IV antibiotics in the hospital and with PICC lines (as seen in the preoperative photo). She decided to undergo removal of both implants to remove the foreign bodies which continued to get infected. Both breasts were then reconstructed with DIEP flaps at the same surgery. She has been free of infection since the DIEP flap breast reconstruction.

Case 31
Description
This is a 64 year old female who was diagnosed with left breast cancer. She decided to undergo bilateral mastectomies and desired DIEP flap reconstruction because she would need radiation to the left breast after mastectomy. She underwent bilateral breast reconstruction with DIEP flaps and the left breast was reconstructed to be slightly larger since this would shrink slightly with radiation therapy. The postoperative photo seen is before radiation therapy. After radiation therapy is completed, she can undergo nipple reconstruction and tattooing.

Description
This is a 55 year old female who had previously undergone bilateral mastectomies for breast cancer. She had reconstruction performed by another plastic surgeon with tissue expanders and then implants. She was not happy with her reconstruction including the overall shape of her breasts, intermittent pain, and the unnatural feel of the implants. She underwent removal of both of the breast implants and replacement with DIEP flaps. She was the perfect candidate and had the right amount of tissue in her lower abdomen to reconstruct both breasts. She subsequently underwent nipple reconstruction and she is scheduled to undergo tattooing.

Description
This is a 47 year old female who was diagnosed with right breast cancer. She decided to undergo double mastectomies, but she would need radiation therapy to the right breast after mastectomy. Understanding the higher risks of implant reconstruction with radiation therapy, she elected to undergo bilateral breast reconstruction with DIEP flaps. She has finished radiation therapy in the postoperative photo seen.

Description
Patient was referred from another hospital after undergoing double mastectomies and tissue expander reconstruction. She unfortunately lost her right breast tissue expander due to infection. She desired to no longer have implants and therefore underwent DIEP flap reconstruction of both breasts. She still needs nipple reconstruction and tattooing.

Description
Patient underwent a right nipple sparing mastectomy. Due to her large breast size, she needed a Stacked DIEP flap, which uses two flaps from both sides of the lower abdomen to reconstruct one breast. One flap is “stacked” on top of the other flap to create a larger breast.

She was very unhappy with her implants, as they were shaped “gummy bear” implants that kept flipping and rotating inside her. She underwent removal of both of her implants and DIEP flap breast reconstruction. She was incredibly pleased with the natural feel of her breasts after flap reconstruction without implants.

Description

Description
This patient underwent bilateral mastectomies and tissue expander reconstruction by another plastic surgeon. She needed right breast radiation therapy, which caused opening of her right breast incision and implant exposure. The right implant was therefore removed and her radiated skin became adherent to her ribs. She was referred for bilateral breast DIEP flaps. All the tight radiated skin from the right chest was removed and replaced with soft non-radiated abdominal skin with the flap procedure.

She then underwent radiation to her left breast which left her with significant capsular contracture (hardening of the implant) and thinning of the skin. She still needs to undergo left nipple and areola 3D tattooing.

Description

She had right breast radiation therapy which made the implant harder and higher on her chest (capsular contracture). She was referred for bilateral implant removal and reconstruction with DIEP flaps. Description
Patient with a history of right breast cancer who had previously undergone tissue expander and implant reconstruction by another plastic surgeon. She had radiation to her right breast and had right breast capsular contracture and bilateral animation deformities (from implants being behind her muscle). She underwent removal of both of her implants, placement of her pectoralis muscles back into the correct position on the chest, and reconstruction with DIEP flaps.
